Assisted Living

Assisted Living facilities are facilities that residents reside in where they are frail but can maintain a certain level of independence requiring minimal assistance for ADLs (Activities of Daily Living), medication management or personal care. These facilities allow residents to enjoy their level of function with as little intrusion as possible. They have access to dining facilities, activities, Physical Rehabilitation  services, day trips, beauty shops and more! Our experienced Physicians & Nurse Practitioners make regular visits to these facilities to medically manage the residents so they do not have to leave the facility for their Primary Care needs. We can order lab tests and X- Rays to manage them safely in the facility.
